29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) Matrix Bagger #12:  An employee was exposed to an approximately 5-foot fall hazard throughout the day while cleaning the hopper with air. The employee was standing on top of the electric panel box that controls the machine, on or about 4/17/24.  b) Matrix Bagger#12 Employee was exposed to a fall hazard of approximately 10' 11" while working on a catwalk without utilizing fall protection. The guardrail was not being utilized, on or about 4/17/24.
