29 CFR 1910.176(b):Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: a) On or about March 15, 2024, the employer did not ensure that a tier of IBC totes was stored in a manner so that they were stable and secure against sliding and collapse. A forklift operator lifted an IBC tote in a first row of IBC totes, which bumped/nudged IBC totes in the second row, causing totes to fall backward into a bulk chemical tank farm, endangering employees to potential stuck-by hazards.
