29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them: a.) Control Room - On or about April 29, 2025, an employee installing a low volume tunnel controller in a motor control panel sustained 3rd degree burns to the right hand when loose wires came in contact with a high voltage breaker (480V), causing an arc flash to occur. The employer failed to ensure that live parts were deenergized before performing service or maintenance, thereby exposing employees to electrical hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: (a) Control Room - On or about April 29, 2025, an employee sustained 3rd degree burns to the right hand when arc flash occurred while the employee performed service and maintenance on a Motor Control Cabinet. The employer failed to ensure that employees exposed to electrical hazards wore appropriate personal protective equipment for the work being performed.
