Control Room - On or about April 29, 2025,  an employee installing a low volume tunnel controller in a motor control panel sustained 3rd degree burns to the right hand when loose wires came in contact with a high voltage breaker (480V), causing an arc flash to occur.  The employer failed to ensure that live parts were deenergized before performing service or maintenance, thereby exposing employees to electrical hazards.
