29 CFR 1910.303(b)(2):29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) at 6801 NE Cornfoot Road, Portland, Oregon 97218, Maintenance Hanger Bldg. 255 on or about 21 March 2024 and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in minor burns and electrical shock. For Reference: Underwriters Laboratories (UL) refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL white book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; they are not to be series-connected to other RPTs or connected to extension cords. Location: Maintenance Hanger Bldg. 255 Equipment: 120-volt 15-amp Relocatable Power Tap Condition: A relocatable power tap that is designed to be plugged into a branch circuit was connected to a 120-volt 15-amp 14-gauge extension cord.
