29 CFR 1910.303(b)(2):Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling. (a)at 6801 NE Cornfoot Road, Portland, Oregon 97218, Bldg. 142, MSG office on or about 21 March 2024 and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power taps (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in minor burns and electrical shock. For Reference: Underwriters Laboratories (UL), refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL white book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; they are not to be series-connected to other RPTs or connected to extension cords.
