29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about March 18, 2024, the employer did not ensure that the heat seal bar on the #8 Assembly Line(Horizontal Form Seal Packer) was guarded to prevent access while the line was in operation.  Employees were exposed to crushing injuries when the heat seal bar cycles, creating a caught between hazard between the front and back bars of the assembly.   b)  On or about March 18, 2024, the employer did not ensure that the cool seal bar on the #8 Assembly Line(Horizontal Form Seal Packer) was guarded to prevent access while the line was in operation.  Employees were exposed to crushing injuries when the cool seal bar cycles, creating a caught between hazard between the front and back bars of the assembly.   c)  On or about March 18, 2024, the employer did not ensure that the perforation blade on the #8 Assembly Line(Horizontal Form Seal Packer) was guarded to prevent access while the line was in operation.  Employee were exposed to laceration injuries when the blade cycles to perforate the packages moving down the assembly line.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
