29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a)Westover Masonry, LLC at 304 15th Avenue, Brookings, SD 57006: On or about and at times prior to February 10, 2024, the employer did not ensure a training program was developed and implemented to ensure designated site personnel were trained in the safe use of Hydro Mobile Scaffold, Serial Number: 5BM21645, platform system to include hazards related to but not limited to, stepping onto and using the equipment's railing as a working surface. This condition exposed employees to fall hazard injuries at heights of greater than 10 feet. (b)Westover Masonry, LLC at 304 15th Avenue, Brookings, SD 57006: On or about and at times prior to February 10, 2024, the employer did not ensure a training program was developed and implemented to ensure designated site personnel were trained in the safe use of Hydro Mobile Scaffold, Serial Number: 5BM2768, platform system prior to use. This condition exposed employees to various hazards associated with the use of the scaffolding. (b)Westover Masonry, LLC at 304 15th Avenue, Brookings, SD 57006: On or about and at times prior to February 10, 2024, the employer did not ensure a training program was developed and implemented to ensure designated site personnel were trained in the safe use of Hydro Mobile Scaffold, Serial Number: 1025599 platform system prior to use. This condition exposed employees to various hazards associated with the use of the scaffolding.
