On or about May 2, 2024, the employer did not ensure that employees were protected from fall hazards when they were elevated on a pallet by a forklift.  Employees were exposed to falls of approximately 10 feet while collecting material for an order.

29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about March 28, 2024, the employer did not develop,  document  and utilize hazardous energy control procedures prior to allowing employees to engage in service and maintenance activities on the Minster Press, Serial no-G1-110-26842. The Press was not locked out as the employee reached into the point of operation to apply lubricating oil to the upper die, there by exposing him to crushing and amputation injuries. 29 CFR  1910.178(l)(4)(ii)(A):Refresher training in relevant topics were not provided to the operator when the operator was observed operating a vehicle in an unsafe manner.  On or about May 2, 2024, the employer failed to ensure that refresher training was provided to an operator who was observed operating a fork truck in an unsafe manner, thereby exposing employees to fall injuries.  A supervisor observed an forklift operator lift an employee 10 feet in the air on a pallet and did not intervene.  The operator was not retrained after this unsafe act was observed. The ingoing nip points created by the metal cutting tool  that performs the thread-cutting did not have any guarding to prevent employees from accessing the zone of danger.  b) On or about May 22, 2024, the employer did not ensure that the point of operations was adequately guarded on the Threading Snow Machine  Model 34 NTRM Serial-41766 to protect against crushing and amputation injuries.  The hinged guard was frequently opened by employees while the machine was operating, making it ineffective to protect employees from the moving parts in the point of operations.  c) On or about May 22, 2024, the employer did not ensure that the point of operations was adequately guarded on the Danly Air Press Model 84S92710017 to protect against crushing and amputation injuries.  The ingoing nip points at the point of operation by the die that  connects the metal parts did not have any guarding to prevent employees from accessing the zone of danger.  d) On or about May 22, 2024, the employer did not ensure that the point of operations was adequately guarded on the Danly Air Press Model 84922701  to protect against crushing and amputation injuries.  The ingoing nip points at the point of operation created by the die  that connects the two metal parts did not have any guarding to prevent employees from accessing the zone of danger.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Such examination shall be made at least daily.  a) On or about May 6, 2024, the employer did not ensure that the Yale Industrial truck- Model GLC050LX2NEAE087, Serial #B967V03247T was examined for conditions that could adversely affect the truck's safety before being placed into service.  Employees operated lift trucks to move various materials throughout the facility.  b) On or about May 6, 2024, the employer did not ensure that the Yale Industrial truck Yale Model GLC050VXNVREAE083, Serial #A910V23976L was examined for conditions that could adversely affect the truck's safety before being placed into service.  Employees operated lift trucks to move various materials throughout the facility.  c) On or about May 22, 2024, the employer did not ensure that powered industrial trucks were examined daily for conditions that could adversely affect the truck's safety. Employee operated the lift truck with the Morse attachment when the locking mechanism failed during the unloading of parts in the Tapping Department.
