29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:  On or about July 11, 2024, the employer did not ensure annual audiograms were provided for employees who were exposed to  noise levels that were equivalent or exceeded the 8 hour time weighted average (TWA) of 85 dBA working in the South Press room:  a.  On or about July 11, 2024, a press operator was exposed to noise at 70.88% of the permissible daily dose, or an average sound level of 88.7 dBA as measured over 410 minutes of sampling.  This dose is equivalent to an 8 hour TWA exposure of 87.5 dBA and included a zero increment for the period of 70 minutes not monitored.   b.  On or about July 11, 2024, a press operator was exposed to noise at 81.62% of the permissible daily dose, or an average sound level of 89.6 dBA as measured over 412 minutes of sampling.  This dose is equivalent to an 8 hour TWA exposure of 88.5 dBA and included a zero increment for the period of 68 minutes not monitored.    c.  On or about July 11, 2024, a press operator was exposed to noise at 145% of the permissible daily dose, or an average sound level of 93.8 dBA as measured over 411 minutes of sampling.  This dose is equivalent to an 8 hour TWA exposure of 92.7 dBA and included a zero increment the period of 69 minutes not monitored.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.95(k)(1):  The employer shall train each employee who is exposed to noise at or above an 8-hour time weighted average of 85 decibels in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program.  On or about May 21, 2024,  the employer did not ensure training was provided on various elements of a hearing conservation program, such as, but not limited to, the effects of noise, hearing protection, and audiometric testing for the employees in the North and South Press Room.  Employees in these areas were exposed to noise at or above an 8-hour time weighted average of 85 decibels.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:  1. Requirement for labeling and other forms of warning; 2.  Employee information and training; 3.  A list of hazardous chemicals known to be present in the workplace; 4.  Methods to inform employees of the hazards on non routine tasks;  On or about May 21, 2024, employees were exposed to respiratory and chemical hazards from chemicals, such as but not limited to, propane gas and Masterlube 4000 HV. The employer did not develop or implement a Hazard Communication program that includes the above mentioned elements.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
