29 CFR 1926.416(a)(1):  Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a)  On or about  March 28, 2024,  employees were exposed to electrical shock while working in close proximity to energized overhead secondary power lines having a voltage rating of 120/240 volts.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On March 28, 2024, employees performing roofing work were exposed to fall hazards of approximately 12 feet.  No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a)  On March 28, 2024, an extension ladder used by employees to gain access to the working surface was not long enough to have it's side rails extend at least three feet above the roof eave.     No abatement certification or documentation is required for this item.
