On or about March 3, 2026, at the above addressed jobsite, an employee performing residential roofing operations, thirteen (13) feet seven (7) inches above the ground, was not protected from falls, thereby exposing employees to fall hazards. b. On or about March 3, 2026, at the above addressed jobsite, employees performing residential roofing operations, eight (8) feet six (6) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards. The Mietus Bros. Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number 1737930, citation number 1, item number 2 and was affirmed as a final order on May 29, 2024, with respect to a workplace located at 1227 Forest Ave, Oak Park, IL 60302.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a. On or about March 3, 2026, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls. The Mietus Bros. Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1) , which was contained in OSHA inspection number 1737930, citation number 1, item number 3 and was affirmed as a final order on May 29, 2024, with respect to a workplace located at 1227 Forest Ave, Oak Park, IL 60302.
