29 CFR 1910.252(a)(2)(vii): Where practicable, all combustibles shall be relocated at least 35 feet (10.7 m) from the work site. Where relocation is impracticable, combustibles shall be protected with flameproofed covers or otherwise shielded with metal or curtains. a) On or about April 2, 2024, at the establishment located at 2057 Bellevue Street, Green Bay, Wisconsin; in the Metal Shop, the employer did not ensure that all combustibles, such as but not limited to cardboard, was relocated at least 35 feet (10.7m) away from the cutting work or protected with flameproofed covers or otherwise shielded with metal or curtains, exposing employees to fire hazards. Cardboard boxes were located less than 4 feet away from the cutting work.

29 CFR 1910.252(a)(2)(ix): Where cutting or welding is done near walls, partitions, ceiling or roof of combustible construction, fire-resistant shields or guards shall be provided to prevent ignition. a) On or about and at times prior to April 2, 2024, at the establishment located at 2057 Bellevue Street, Green Bay, Wisconsin; in the Metal Shop, the employer did not ensure that fire-resistant shields or guards were provided used to prevent ignition of combustible construction during plasma cutting, exposing employees to fire hazards. Plywood walls were located less than 5 feet away from the cutting work.

29 CFR 1910.252(b)(2)(i)(A): Helmets or hand shields shall be used during all arc welding or arc cutting operations, excluding submerged arc welding. Helpers or attendants shall be provided with proper eye protection. a) On or about and at times prior to April 2, 2024, at the establishment located at 2057 Bellevue Street, Green Bay, Wisconsin; in the Metal Shop, the employer did not ensure that arc cutting operations were enclosed by an individual booth, noncombustible or flameproof screens, or requiring the use of appropriate goggles to protect employees from ultraviolet radiation generated by the arc cutting operations, exposing employees to the hazards associated with ultraviolet radiation.
