Working space required by this standard may not be used for storage: (a) On and before April 2, 2024, and at times prior, the employer did not maintain workspace of at least 3 feet of space in front of electrical panels located at the north wall in that the panels were blocked by debris, materials, and equipment.

29 CFR 1910.334(a)(2)(ii): Portable cord and plug connected electrical equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) On or before April 02, 2024, and at times prior, the employer did not visually inspect flexible power cord for external defects and for evidence of possible internal damage. An extension cords was discovered with damage, pulled insulation. The extension cord was used as a multi-purpose power cord for many different tasks. This condition exposed the employees to electrical shock hazards.
