1910.212(a)(3)(iv)(a) Guillotine cutters. 29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) On or about March 19, 2024, with respect to a worksite located at 411 County Highway PP, Plymouth, WI 53073, employees were exposed to potential caught-in and struck-by hazards when 640-Block Cutter (MG881) was not guarded to prevent access to the point of operation. b) On or about July 11, 2024, with respect to a worksite located at 411 County Highway PP, Plymouth, WI 53073, employees were exposed to potential caught-in and struck-by hazards when 640-Block Cutter (MG2060) was not guarded to prevent access to the point of operation. c) On or about July 11, 2024, with respect to a worksite located at 411 County Highway PP, Plymouth, WI 53073, employees were exposed to potential caught-in and struck-by hazards when 640-Block Cutter (MG2770) was not guarded to prevent access to the point of operation. d) On or about July 11, 2024, with respect to a worksite located at 411 County Highway PP, Plymouth, WI 53073, employees were exposed to potential caught-in and struck-by hazards when 640-Block Cutter (MG1855) was not guarded to prevent access to the point of operation. e) On or about July 11, 2024, with respect to a worksite located at 411 County Highway PP, Plymouth, WI 53073, employees were exposed to potential caught-in and struck-by hazards when 640-Block Cutter (MG2399), on Line H1, was not guarded to prevent access to the point of operation. f) On or about July 11, 2024, with respect to a worksite located at 411 County Highway PP, Plymouth, WI 53073, employees were exposed to potential caught-in and struck-by hazards when 640-Block Cutter (MG2197) was not guarded to prevent access to the point of operation.
