1910.212(a)(3)(ii)  The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about June 18, 2024, at an establishment located at 411 County Highway PP in Plymouth, Wisconsin, on the U2 shredded cheese line; employees cleaning around the Loos anti-cake machine (Serial Number: 0224-3613) were exposed to amputation hazards associated with the rotating auger when the machine was running without its guards in preparation for cleaning. The employer did not ensure employees implemented energy control application steps prior to allowing them to clean near and under the machine when it was removed from its guarding.   As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  At an establishment located at 411 County Highway PP in Plymouth, Wisconsin:  b) On or about July 11, 2024, on the mezzanine level of SL1 and SL2, employees were exposed to caught-in and amputation hazards to the hands and fingers associated with the in-going nip points created by the unguarded film infeed rollers on the two Zip Pak plastic film packaging machines. The machines were not effectively guarded to prevent exposure to these hazards.   c) On or about June 17, 2024, to June 21, 2024, in the maintenance and machine shop area, employees were exposed to entanglement hazards created by the unguarded chuck on the JET geared head engine metal lathe. The machine was not effectively guarded to prevent exposure to these hazards.   d)  On or about June 10, 2024, to June 14, 2024, in the maintenance and machine shop area, employees were exposed to entanglement and struck-by hazards created by the unguarded chuck and flying metal chips and sparks from the JET turret milling machine. The machine was not effectively guarded to prevent exposure to these hazards.   e) On or about June 10, 2024, to June 14, 2024, in the maintenance and machine shop area, employees were exposed to entanglement hazards created by the unguarded chuck on the JET belt drive bench lathe. The machine was not effectively guarded to prevent exposure to these hazards.
