29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure guarding was in place on the Clausing Drill Press, Model: 2223, Serial: 109378 within the medium/vertical area. This condition exposed employees to caught-in hazards.  (b)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure guarding was in place on the Duracraft 16 Speed Drill Press, Model: 45160, Serial: G14437 within the small vertical area. This condition exposed employees to caught-in hazards.  (c)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure guarding was in place on the Central Machinery 16 Speed Heavy Duty Drill Press, Model No. T-583 within the small vertical area. This condition exposed employees to caught-in hazards.  (d)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure guarding was in place on the Bridgeport drill press, Serial No. 2J-53886, located in the tool area. This condition exposed employees to caught-in hazards.  (e)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure that four CNC-milling machines (Okuma Machine, Type: MC-4VAE including Serial No. 9724), located in the small vertical area, was properly guarded to prevent employees from having any part of their body in the danger zone during machine operations. This condition exposed employees to caught in and struck by hazards.

This condition exposed employees to amputations and caught in hazards.   (b)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure guarding was in place on the horizontal bandsaw, Amada Model HA250, Serial No. Serial No. 456618, located in the saw area. This condition exposed employees to amputations and caught in hazards.   (c)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure guarding was in place on the horizontal bandsaw, Amada Model HA250, Serial No. Serial No. 156618, located in the saw area. This condition exposed employees to amputations and caught in hazards.  (d)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure guarding was in place on the horizontal bandsaw, Amada, Model HA400W, Serial No. 45732049, located in the saw area. This condition exposed employees to amputations and caught in hazards.  (e)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure guarding was in place on the horizontal bandsaw, Amada, Model 230, Serial No. 45732050, located in the saw area. This condition exposed employees to amputations and caught in hazards.  (f)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure that a 55-Ton Hydraulic Shop Press located in the grinding area was adequately guarded at the point of operation. This condition exposed employees to amputation, nip-point, struck-by and crushing hazards.  (g)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure that a lathe Barber-Coleman, Machine 14-30, Serial No. 1266, located in the north end of the building, was adequately guarded at the point of operation. This condition exposed employees to caught-in and nip-point hazards.  (h)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure that a lathe Barber-Coleman, Machine 14-15, Serial No. 1018, located in the north end of the building, was adequately guarded at the point of operation. This condition exposed employees to caught-in and nip-point hazards.  (i)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure that lathe Barber-Coleman, adjacent to Barber-Machine 14-15, located in the north end of the building, was adequately guarded at the point of operation. This condition exposed employees to caught-in and nip-point hazards.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  (a)Dynamic Engineering, Inc. at 221 Cessna Street, Watertown, SD 57201: On or about and at times prior to April 3, 2024, the employer did not ensure that space about electric equipment and or sufficient access and working space was provided and maintained from the front of an electrical panel box within the north end of the building.   This condition exposed employees to electrical hazards due to the blocked panel.
