29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):   (a)Dynamic Engineering, Inc. at 720 11th Avenue SE, Watertown, SD 57201: On or about and at times prior to April 24, 2025,  the employer did not establish a program consisting of energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative, including but not limited to the following (as required by the standard):   (1) Horizontal Saws (2) Vertical Saws (3) Drill Presses (4) Flex Arm Tappers (5) Sanders (6) Industrial Air Compressor  (7) Band Saws  This condition exposes authorized and affected employees to shock / electrocution hazards.  Abatement Note: As part of an energy-control program, employers must:  (1) Establish energy-control procedures for removing the energy supply from machines and for putting appropriate lockout or tagout devices on the energy-isolating devices to prevent unexpected reenergization. When appropriate, the procedure also must address stored or potentially reaccumulated energy.   (2) Train employees on the energy-control program, including the safe application, use, and removal of energy controls; and   (3) Inspect these procedures periodically (at least annually) to ensure that they are being followed and that they remain effective in preventing employee exposure to hazardous energy.   The energy-control procedures must outline the scope, purpose, authorization, rules, and techniques that employees will use to control hazardous energy sources, as well as the means that will be used to enforce compliance. These procedures must provide employees at least the following information:   (1) A statement on how to use the procedures;  (2) Specific procedural steps to shut down, isolate, block, and secure machines;   (3) Specific steps designating the safe placement, removal, and transfer of lockout/tagout devices and identifying who has responsibility for the lockout/tagout devices; and   (4) Specific requirements for testing machines to determine and verify the effectiveness of lockout devices, tagout devices, and other energy-control measures.

29 CFR  1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   (a)Dynamic Engineering, Inc. at 720 11th Avenue SE, Watertown, SD 57201: On or about and at times prior to April 24, 2025, the employer did not ensure guarding was in place on the Rockwell,  DoAll Saw, Contour Band Saw, Model 2013-V serial No. 4632-A, located in the large area, in that a belt and pulley were exposed due to an open panel on the side of the machine.  This condition exposed employees to amputations and caught in hazards.

29 CFR  1910.242(b): Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment:   (a)Dynamic Engineering, Inc. at 720 11th Avenue SE: On or about and at times prior to April 24, 2025, the employer did not ensure that compressed air used for cleaning was reduced to less than 30 p.s.i or chip guarding controls in place.  Pneumatic air nozzles were identified throughout the plant, to include, but not limited to equipment such as the MH 63 Mori Seiki Horizontal Mill located in the Large Parts section.  This condition exposed employees to struck-by hazards and pulmonary embolism.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a)Dynamic Engineering, Inc. at 720 11th Avenue SE, Watertown, SD 57201: On or about and at times prior to April 24, 2025,  the employer did not ensure guarding was in place on the Bridgeport drill press, Series I - 2HP,  Model #: F 7347-00-964, Serial #.: 1-18-017, located in the Large Horizontal Equipment Area. This condition exposed employees to caught-in hazards.   (b)Dynamic Engineering, Inc. at 720 11th Avenue SE, Watertown, SD 57201: On or about and at times prior to April 24, 2025,  the employer did not ensure guarding was in place on the Enco drill press located in the Medium Horizontal Equipment Area. This condition exposed employees to caught-in hazards.  Dynamic Engineering Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1739191, citation number 1, item number 1-a and was affirmed as a final order on August 27, 2024, with respect to a workplace located at 221 Cessna Street, Watertown, SD 57201.

This condition exposed employees to amputations and caught in hazards.  Dynamic Engineering, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1739191, citation number 1, item number 1b and was affirmed as a final order on August 27, 2024, with respect to a workplace located at 221 Cessna Street, Watertown, SD 57201.
