29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) On or about April 4, 2024, at the worksite: employees were exposed to 22 foot fall hazards while conducting roofing work on a 4/12 pitched roof, in that, fall protection was not used.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) On or about April 4, 2024, at the worksite: the employer did not prepare and maintain a written fall protection training program certification for employees exposed to fall hazards of 22 feet while working on the roof.
