29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) 458-564 Baxter Lane East St, Bozeman, MT: On or about April 4, 2024, and at times prior, employees were exposed to catapult and subsequent fall hazards while operating and riding in an Genie S-65 Aerial Lift without being connected to the boom or basket.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 458-564 Baxter Lane East St, Bozeman, MT: On or about April 4, 2024, and at times prior, employees were exposed to fall hazards of approximately 10 feet to the ground below while installing roofing materials onto an eve without the use of fall protection.
