If such hazards are present, or likely to be present, the employer shall:   a. Located at 15 North Park Avenue, Wellston, Ohio 45692: On or about April 5, 2024, the employer did not assess the workplace to include but not limited to various pin spotter machines, landscaping and processes that included the handling of chemicals to determine if hazards were present which would necessitate the use of  personal protective equipment.  Employees that perform work on mechanical equipment,  perform facility landscaping and utilize various chemicals for cleaning of the equipment without the workplace conditions being assessed to determine the need for and type of PPE needed.

The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a. Located at 15 North Park Avenue, Wellston, Ohio 45692: On or about April 5, 2024, the employer did not establish a program for the control hazardous energy that included energy control procedures, employee training, or periodic inspections. Employees perform regular maintenance, cleaning and clearing of jams on pin spotter machines without a hazardous energy control program in place which exposes the employees to potential hazards associated with the unexpected equipment startup or release of stored energy.

The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a. Located at 15 North Park Avenue, Wellston, Ohio 45692: On or about April 5, 2024, employees were exposed to machine hazards associated with moving parts, pinch points, and electrical shock hazards during servicing and maintenance tasks on the pin spotter machines. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect plug [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control element, involving verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
