29 CFR  1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.  (a)  On or about April 5, 2024, at a new construction residential jobsite located at 499 Kiddle Ln. Union Grove WI, an extension  ladder erected on the East side of the structure, at, and in front of the open front door of the structure, was being used as the primary source of access to the 6:12 pitched roof. This ladder was erected in a location where it could be displaced by normal work activities creating a fall and struck-by hazard.

29 CFR  1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear:  a)  On or about April 5, 2024, at a new construction residential jobsite located at 499 Kiddle Ln. Union Grove WI. an extension  ladder was erected on the East side of the structure, at, and in front of the open front door of the structure. This ladder was being used as the primary source of access to the 6:12 pitched roof and the base of the ladder was blocked by extension cords and building material debris. This created a trip and fall hazard to employees using the ladder for access to and egress from the roof of the structure.

29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about April 5, 2024, at a new construction residential jobsite located at 499 Kiddle Ln. Union Grove WI, the employer who was the competent person, did not ensure frequent and regular inspections of the job site were conducted to identify hazards and ensure the job site was maintained safe.  JC Siding, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1674359, citation number 1, item number 1, and was affirmed as a final order on 1/11/2024, with respect to a workplace located at 9714 Bergamont Drive, Franklin, WI 53132.  JC Siding, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1643732, citation number 1, item number 1, and was affirmed as a final order on 3/24/2023, with respect to a workplace located at 1825 Yorkville Meadows, Union Grove, WI 53182.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) On or about April 5, 2024, at a new construction residential jobsite located at 499 Kiddle Ln. Union Grove WI, an extension  ladder erected on the East side of the structure, at the front door, being used for access to the 6:12 pitched roof, did not extend at least three (3) feet above its upper contact point, and was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof to a fall hazard of approximately 17 feet, 4 inches onto a gravel covered surface below.  (b) On or about April 5, 2024, at a new construction residential jobsite located at 499 Kiddle Ln. Union Grove WI, an extension  ladder erected on the North side of the structure, and being used for access to the 6:12 pitched roof, did not extend at least three (3) feet above its upper contact point, and was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof to a fall hazard of approximately 13 feet, 4 inches onto a gravel and dirt surface below.  JC Siding, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1674359, citation number 1, item number 3, and was affirmed as a final order on 1-11-2024 , with respect to a workplace located at 9714 Bergamont Dr. Franklin, WI. 53132.
