29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) Worksite, 1966 Broadhollow Road, Building E, in Farmingdale, NY, 11735 - Employees on site work working in the shipping area of the storage warehouse in close proximity to an electrical sub-contractor on a scissor lift approximately 20-feet in the air. The employees on the ground were not provided with a protective helmet to prevent them from being injured by falling objects; on or about April 2, 2024. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.179(b)(6)(i): A minimum clearance of 3 inches overhead and 2 inches laterally was not provided and maintained between crane(s) and obstructions: a) Worksite, Shipping Area 1966 Broadhollow Road, Building E, in Farmingdale, NY, 11735 - Bridge Crane 3-ton Spantect Single Girder Top Running Dual Motor system with a Robbins and Myers 460V 3-ton Electric Wire Rope Hoist. The overhead bridge crane was not provided with two inches of lateral clearance between the bridge and a Genie GS 3232 scissor lift; on or about April 2, 2024. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
