29 CFR  1910.37(a)(4):Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  (a) at 3502 U.S. Highway 30, La Grande, Oregon 97850, Warehouse building, on or about 10 April 2024 and times prior thereto: Employees were exposed to fall/trip hazards  from exit route emergency light not operating properly which would impair employee vision in the event of an emergency, resulting in injuries, bruises, and minor disabilities.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.37 (a) (4), which was contained in OSHA inspection number 1702562, notice number 3, item number 1 and was affirmed as a final order on 12/02/2023, with respect to a workplace located at U.S. Forest Service RR- Siskiyou NF, High Cascades Ranger District, 47201 Highway 62, Prospect, Oregon.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks:  (a) at3502 U.S. Highway 30, La Grande, Oregon 97850, Warehouse building, on or about 10 April 2024 and times prior thereto: Employees were exposed to struck by and caught in hazards such as flying chips and clothing caught in rotating parts. Employer failed to ensure that the drill press was equipped with a guard sufficient to protect the operator from the rotating chuck and flying debris created during operation.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212 (a) (1), which was contained in OSHA inspection number 1702908, notice number 3, item number 7 and was affirmed as a final order on 12/126/2023 with respect to a workplace located at Forest Service, Wild River Ranger District, Cave Junction, Oregon.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  (a) at 3502 U.S. Highway 30, La Grande, Oregon 97850, Fire management area, Union Hot Shot building, on or about 10 April 2024 and times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in minor burns and electrical shock.   The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303 (b) (2), which was contained in OSHA inspection number 1702562, notice number 3, item number 4 and was affirmed as a final order on 12/06/2023 with respect to a workplace located at Forest Service RR-Siskiyou NF, High Cascades Ranger District, Prospect, Oregon.  For Reference:  Underwriters Laboratories (UL) refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL white book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; they are not to be series-connected to other RPTs or connected to extension cords.
