29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  (a) On or about April 10, 2024 at 2232 Seminole St, Grafton, WI 53024, the employer did not ensure that all employees conducting cleanup of the roofing materials being thrown from the roof were wearing protective helmets thus exposing them to struck by hazards.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about April 10, 2024, at 2232 Seminole St, Grafton, WI 53024, employees were using staplers to install leak barrier on the roof of an existing condominium complex without eye protection. This exposed employees to a struck by hazard and eye impalement injury.  (b) On or about April 10, 2024, at 2232 Seminole St, Grafton, WI 53024, employees were conducting cleanup operations on ground level without wearing proper eye protection. This exposed employees to flying particles and potential eye injury.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about April 10, 2024, at 2232 Seminole St, Grafton, WI 53024, employees conducting roofing operations on an existing residential condominium complex with an approximately 9:12 pitch were not wearing fall protection. This exposed employees to a fall hazard of approximately 24 feet from roof to the ground below on the front (south) side of the home.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) On or about April 10, 2024, at 2232 Seminole St, Grafton, WI 53024, the employer did not ensure employees installing a roof on an existing condominium complex were properly wearing a full body harness that had a lifeline connected to the back dorsal D-Ring.  Employees tied the lifeline to the front D-Ring  of the body harness, exposing employees to a fall hazard of approximately 24 feet from roof to the ground below on the front (south) side of the home.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) On or about April 10, 2024, at 2232 Seminole St, Grafton, WI 53024, employees were accessing the roof to conduct roofing work from two different ladders (side of home and back of home) which did not extend at least three feet above the landing surface.
