29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about September 19, 2024, at a jobsite located at 9731 Bluegrass Place, OAK CREEK, WI 53154, employees conducting roofing activities (shingle installation) on a 8:12 pitch roof of the residential structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the roof of the structure of approximately 9.5 feet. Serrato Brothers Contractors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1926.501(b)(13) , which was contained in OSHA inspection number 1740407, citation number 1, item number 2 and was affirmed as a final order on May 22 2024, with respect to a workplace located at 2232 Seminole St, Grafton, WI 53024,

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On or about September 19, 2024, at a jobsite located at 9731 Bluegrass Place, OAK CREEK, WI 53154, the extension ladder being used by employees to access the roof of the structure at or near the garage door, and the front door did not extend at least three feet above the upper point of contact with the eave, nor was the ladder secured in place. This exposed employees accessing and egressing the roof, to a fall hazard of approximately 9.5 feet onto building materials and concrete. Serrato Brothers Contractors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1926.1053(b)(1) , which was contained in OSHA inspection number 1740407, citation number 1, item number 3 and was affirmed as a final order on May 22 2024, with respect to a workplace located at 2232 Seminole St, Grafton, WI 53024,
