29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Facility Wide: On April 11, 2024, the employer did not establish a lockout tagout program consisting of energy control procedures and employee training to protect employees during service and repair tasks such as but not limited to saw blade changes on the Amada 250W horizonal bandsaw.  (b) Facility Wide: On April 11, 2024, the employer did not establish a lockout tagout program consisting of energy control procedures and employee training to protect employees during service and repair tasks such as but not limited to saw blade changes on the Amstrong Blum Serial #8959A bandsaw.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Manual Machine Area - On April 11, 2024, the employer did not ensure that the 13/40 Standard Modern lathe was equipped with an enclosure guard to protect employees from the rotating chuck during operation.  (b) Manual Machine Area - On April 11, 2024, the employer did not ensure that the Maxturn 2140 lathe was equipped with an enclosure guard to protect employees from the rotating chuck during operation.   In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):  (a) Engraving Department: On or about April 11, 2024, the employer failed to ensure pulleys equipped on the Amstrong Blum Serial #8959A bandsaw within seven feet from the floor were provided with guards on all exposed sides.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
