29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a) Engraving Department - On or about March 24, 2025, the employer did not ensure pulleys equipped on the Armstrong Blum Serial #89594 bandsaw, on the bottom and side of the bandsaw within seven feet from the floor were provided with guards on all exposed sides. DMS, Inc., was previously cited for a violation of this occupational safety and health standard 1910.219(d)(1), which was contained in OSHA inspection number 1740592, citation 1, item 3a, and was affirmed as a final order on July 17, 2024, with respect to a workplace located at 1120 Ensell Road, Lake Zurich, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven -7 feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt: a) Engraving Department - On or about March 24, 2025, the employer did not ensure a horizontal belt equipped on the side of Armstrong Blum Serial #89594 bandsaw within seven feet from the floor was completely guarded on the inside of the belt and pulley. DMS, Inc., was previously cited for a violation of this occupational safety and health standard 1910.219(e)(4), which was contained in OSHA inspection number 1740592, citation 1, item 3b, and was affirmed as a final order on July 17, 2024, with respect to a workplace located at 1120 Ensell Road, Lake Zurich, Illinois.
