No form of guarding was used to protect employees from amputation hazards caused by the exposed and unused part of the blade. b) On or about March 22, 2024, the employer did not ensure that employees were protected from amputation injuries while operating the Jet Saw in the shop area. No form of guarding was used to protect employees from amputation hazards caused by the exposed and unused part of the blade.
