NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign. 29 CFR  1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Shop Assembly Area - On or about April 2nd, 2025, employees were exposed to the chemical hazards of Phenicon HS Expoxy Tank Lining (flammable, corrosive, sensitizer, carcinogen), while painting the interior of a coolant storage tank / filtration system tank.  Previous training was not effective because it did not cover the physical and health hazards of the paint in use or the protective measures employees could take while using this paint.  Additionally, the training was not delivered in a language that the employees understood.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.184(c)(4):Sling(s) were loaded in excess of their rated capacity:  (a) Shop Assembly Area -On or around March 24, 2025, a steel structure filtration system tank was lifted from a delivery vehicle using two overhead 25-ton gantry cranes and two sets of steel chains. During the lift, the chains were overloaded beyond their rated capacity. As a result, one set of chains broke, causing the load to drop, thereby exposing employees to struck-by and crushing hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
