29 CFR 1910.184(c)(4):Sling(s) were loaded in excess of their rated capacity: (a) Shop Assembly Area -On or around March 24, 2025, a steel structure filtration system tank was lifted from a delivery vehicle using two overhead 25-ton gantry cranes and two sets of steel chains. During the lift, the chains were overloaded beyond their rated capacity. As a result, one set of chains broke, causing the load to drop, thereby exposing employees to struck-by and crushing hazards.
