On or about April 10, 2024, in the Main ammonia process, the walking surfaces in front of the maintenance shop located near the High-Pressure Receiver vessel were not maintained free of hazards such as electrical cords, broken crate and/or blocked by welding machines, manual pallet jacks.

On or about April 10, 2024, in the Main ammonia engine room, the employer did not ensure the Pilot Receiver (PLR1) and ammonia piping and equipment were properly labeled according to their adopted RAGAGEP (IIAR Bulletin 114 Edition 2019 Section 4.1 Piping Markers. and Section 4.2 Component Markers). Instance a) C-7 HSD piping and HTRL and HL piping on HTR-2. Instance b) PLR1 (Pilot receiver).

On or about April 10, 2024, in the Main ammonia engine room, the employer did not ensure dual relief valves on Low Stage Suction Accumulator (LSSA) were replaced and/or recertified according to their employed RAGAGEP including but not limited to IIAR Bulletin 110 (Start Up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems).
