On or about April 9, 2024, in Alky Unit 22 at Phillips 66 Company located in Borger, Texas; the employer did not document that equipment complies with their adopted RAGAGEP, ASME Boiler and Pressure Vessel Code, Section VIII Division 1, 1968 Edition, Appendix M, Section UA-354 (a) (Stop Valves Between Pressure Relieving Device and Vessel) to ensure that  block valve on the inlet of the relief valve R-326 on the Depropanizer Condensate Keg (95- RA06) was locked or sealed in the open position.
