1926.451(a)(1) Each scaffold and scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.   a) On April 22, 2024, at the above addressed jobsite, employees were exposed to the hazard of falling approximately 13 feet when climbing on the railings and crossbraces to travel in and out of the lift of Skyjack Scissors Lift SJ6832 RT.  No abatement certification or documentation is required for this item.

29 CFR 1926.602(d):  The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  29 CFR 1910.178(l)(1)(i):  The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a)  On April 22, 2024, at the above addressed jobsite, the employer did not train and evaluate employees operating the JLG Telehandler, Model G5-18A all-terrain forklift.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
