29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. At a temporary residential job site, located at 277 Tallulah Avenue, Jacksonville, FL 32208:  On or about April 23, 2024, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns to waterproofing paper.   Joseph Marvin Home Repairs, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1673811, Citation Number 2, Item Number 1 and was affirmed as a final order on July 21, 2023, with respect to a workplace located at 9483 Balsam Fir Court - Lot 282, Jacksonville, FL 32208.  Joseph Marvin Home Repairs, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1536092, Citation Number 1, Item Number 1 and was affirmed as a final order on October 20, 2021, with respect to a workplace located at 13998 Nations Eagle Lane, Jacksonville, FL 32226.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system:    a.  On a 4:12 pitched roof of a temporary commercial job site, located at 277 Tallulah, Jacksonville, FL 32208:  On or about April 23, 2024, the employer exposed employees to fall hazards ranging from 8.5 to 19-foot, in that fall protection was not utilized by employees while conducting roofing work.  Joseph Marvin Home Repairs, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1673811, Citation Number 2, Item Number 2 and was affirmed as a final order on July 21, 2023, with respect to a workplace located at 9483 Balsam Fir Court, Lot 282 - Timber Oaks, Jacksonville, FL 32208. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. On a temporary commercial job site located at 277 Tallulah Avenue, Jacksonville, FL 3228: On or about April 23, 2024, the employer exposed employees to fall hazards, in that a 16-foot portable extension ladder was not extended 3-foot above the roof eave as employees utilized the ladder to climb to and descend from the roof work area.   Joseph Marvin Home Repair, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1673811, Citation number 01, Item Number 2-a and was affirmed as a final order on July 21, 2023, with respect to a workplace located at 9483 Balsam Fir Court, Lot 282  - Timber Oaks, Jacksonville, FL 32208.
