29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  (a) In the debarker area of the facility: On or about April 14, 2024, and times prior, employees are exposed to an ingoing nip point while stepping on or over the moving log trough conveyor to access the log deck. The chains of the log trough conveyor were not guarded exposing employees to caught-in hazards.

29 CFR  1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces.  (a) In the debarker area of the facility: On or about April 14, 2024, and times prior, employees step over a 20 inch log trough to access the log deck on the other side. Employees are exposed to caught-in or struck by hazards while climbing over the log trough due to moving chains and lack of walking bridge between the two sides.
