29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure: On or about April 25, 2024, employees were exposed to fall hazards above 6 feet without fall protection while roofing on a residential construction site in Creve Coeur, Illinois. JUAN RAMOS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1709049, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 22, 2024, WITH RESPECT TO A WORKPLACE LOCATED AT 1359 WOODLAND KNOLLS ROAD, METAMORA ILLINOIS.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface to which the ladder was used to gain access: On or about April 26, 2024, employees were exposed to a fall hazard of approximately 9 feet while descending an extension ladder leaned up against the structure because the side rails did not extend at least three feet above the landing surface.
