29 CFR  1910.178(l)(1)(ii):The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  a.) At the worksite, located at 12871 Trade Way Dr Suite #3, Bonita Springs, FL 34135; On or about 4/30/24, the employer exposed employees to struck by hazards, in that, forklift operations were conducted without formal instruction, hands on trainings, evaluations or certifications in place while moving merchandise and products inside and the back of the warehouse.

29 CFR  1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a.) At the worksite, located at 12871 Trade Way Dr Suite #3, Bonita Springs, FL 34135; On or about 4/30/24, the employer exposed employees to fire and electrical shock hazards, in that, the front of electrical panels, located in the warehouse, were blocked by, pallets, paint, propane cylinders and a dolly, where employees move, work, and retrieve merchandise.

At the worksite, located at 12871 Trade Way Dr Suite #3, Bonita Springs, FL 34135; On or about 4/30/24, the employer did not develop, implement and/or maintain at the workplace a written hazard communication program, where employees use chemicals such as All Pro paint thinner, denatured alcohol, methyl ethyl ketone, and M1- advanced mildew treatment while mixing paints and packing containers for customers.
