One employee was observed riding on, while hanging onto the operator's side of a Loadall 508-50 rough terrain forklift while in motion on the residential construction jobsite.

Five employees were working directly below modular home sections which were being hoisted and lifted overhead by a Grove TMS 9000 hydraulic truck crane, and were untrained in proper safe signaling for the crane operations.
