(a) On or about April 30, 2024, at a jobsite located at W197 N16578 Aspen Dr Jackson, WI 53037, employees tearing off old shingles on an approximately 4:12 pitch roof were not wearing fall protection. This exposed employees to a fall hazard of approximately eight feet five inches (8.5) feet from roof to the ground below.
