29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) 6276 Leaping Sheep Way, Longmont, CO 80501: On April 29, 2024, and at times prior, employees were exposed to falls of approximately 11 feet to the ground below while conducting framing activities on the second level of a new residential home.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  (a) 6276 Leaping Sheep Way, Longmont, CO 80501: On April 29, 2024, and at times prior, employees were exposed to falls of approximately 11 feet to the ground below while conducting framing activities on the second level of a new residential home.  The employer failed to provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured:  (a) 6276 Leaping Sheep Way, Longmont, CO 80501: On April 29, 2024, and at times prior, employees were exposed to falls of approximately 11 feet to the ground below while accessing/egressing upper levels of the existing residential home. The ladders were not secured at their top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladders.
