29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Dollar Tree Distribution Center located at 300 Cardinal Drive, Marengo, Ohio, 43334: On or about April 28, 2024, the employer failed to ensure machine specific lockout tagout procedures were implemented for employees tasked with performing maintenance or servicing tasks such as, but not limited to, cleaning and preventative maintenance activities on the PTO chain drive on the Mod 21 (level A) Honeywell Intelligrated conveyor system, exposing employees to amputation hazards.
