29 CFR 1910.95(c)(1):The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) Venture Stone, LLC: On or about May 6, 2024, and at time prior, an employee was exposed to noise levels of 88.7 decibels on an A scale (dBA) while operating stone masonry saw machines. The employee was sampled for 468 minutes being exposed to a dose of 86.3%. Per 29 CFR 1910.95(c)(2) an 8 hour time weighted average of 85 decibels or a dose of fifty percent shall also be referred to as the action level.

29 CFR 1910.305(g)(1)(iv)(D):Flexible cords and cables not specifically permitted otherwise in paragraph (g)(1)(ii) of this section, were used where attached to building surfaces: a) Venture Stone, LLC: On or about May 6th, 2024 and at times prior, employees were exposed to electric shock hazards while using an extension cord attached to the building to operate a stone masonry machine.
