The employer did not ensure the the energy control procedures clearly and specifically outlined steps for shutting down, Isolating, blocking and securing machines or equipment to control hazardous energy for the Knife Gate (SN: 11-OG4087/ MN: OG400) that is part of Line 1 in the hopper area on the mezzanine. 29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about 5/1/2024, an employee was exposed to hazardous energy while conducting service work on a Knife Gate (SN: 11-OG4087/ MN: OG400) due to moving parts of the machine. The employees did not take the steps necessary to control the hazardous energy and isolate the machine prior to removing a guard and preforming maintenance on the Knife Gate Machine. Amended to: The classification of Citation 1, Item 1b shall remain serious as issued. The ADV is amended to ?On or about 5/1/2024, a maintenance employee was troubleshooting a Knife/Slide Gate and did not ensure the surrounding mezzanine area was clear of affected employees pursuant to internal company procedures.? Citation 1, Item 1(b) shall be amended to a section 5(a)(1) violation of the OSH Act.
