Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from hazards associated with struck by hazards:  Extrusion- Employees loading, unloading, and working around Industrial steel storage racks with the use of powered industrial trucks were not protected from struck by hazards where the employer did not identify and post the capacities of the steel storage racks, properly anchor steel storage racks to the floor, remove damaged storage racks from service, and provide a storage rack design professional to evaluate damaged storage rack components.

29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  Upper Mezzanine - Employees working from the mezzanine and employees performing maintenance work on extrusion equipment were not protected from a fall of approximately 8 feet where extrusion equipment was removed, leaving a floor opening that was not guarded.
