29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components are operationally intact. (a) Located at the boiler: On or about May 3, 2024, employees were exposed to thermal hazards, in that the employer did not require them to ensure the seal and the lower man way door on the boiler was operationally intact before bringing the boiler online.
