29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  (a) Room I-020, on or about 9 May 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. Resulting in trip/fall and burn injuries from trying to exit building during an emergency event.  (b)Engineering section, near plotter, on or about 9 May 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. Resulting in trip/fall and burn injuries from trying to exit building during an emergency event.  (c) First floor restroom, on or about 9 May 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. Resulting in trip/fall and burn injuries from trying to exit building during an emergency event.  (d) Building back exit, on or about 9 May 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. Resulting in trip/fall and burn injuries from trying to exit building during an emergency event.  (e) Room 2-124, on or about 9 May 2024 and at times thereof: Employees were exposed to an exit route path lighting that is not operational. 29 CFR 1910.303 (b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling.   (a) Coo conference room, on May 9, 2024, and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in minor burns and electrical shock.   (b) Gym area, on May 9, 2024, and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in minor burns and electrical shock.   (c) Office 2-013, on May 9, 2024, and at times prior thereto: Employees were exposed to fire and electrical shock hazardous condition from a Relocatable power tap (power strips) were series-connected (daisy-chained) to extension cords or other power strips resulting in minor burns and electrical shock.   For Reference:  Underwriters Laboratories (UL) refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL white book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; they are not to be series-connected to other RPTs or connected to extension cords.             Equipment:            120-volt 15-amp Relocatable Power Tap           Condition:               A relocatable power tap that is designed to be plugged into a branch circuit was connected to a 120-volt 15-amp 14-gauge extension cord.
