29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  a) Warehouse,: On or about May 10, 2024, and times prior, the employer failed to ensure that Lock Out/ Tag Out (LOTO) procedures were developed, documented, and utilized by employees tasked with conducting jam clearing activities , on the conveyor belt system at the facility. Employees tasked with  clearing jams on  the conveyor system were exposed to amputation hazards.
