29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about May 7, 2024, the employer did not ensure that a program for the control of hazardous energies was in place at the facility. Employees are tasked with the service and maintenance of equipment including but not limited to babbit machines, CNC machines, air compressors, and other equipment. Hazardous energies present on equipment includes but is not limited to mechanical, electrical, pneumatic, and hydraulic.

Employees operated the powered industrial vehicles as needed prior to completion of training and evaluations.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 7, 2024, the employer did not ensure that effective machine guarding was in place on CNC machines located and operated within the facility. CNC machines either contained door interlock mechanisms which did not function or were bypassed to allow the doors to be open during production. CNC machines observed without working interlocks include but are not limited to: a) NL- 2500 Sy #1, b) NL-3000 SY #3, c) Leadwell V-40 it, d) Amera- Seiki T-415 e) VMC07 Storm f) Suprema 650
