29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a)  Worksite; 420 Maple Avenue, Swissvale, PA -  Employees were removing, repairing and replacing layers of shingles on the second level low pitch roof without the use of a personal fall arrest systems, or an alternative fall protection measure nor was a protected by guardrail systems or safety net systems in place.  Employees were exposed to an approximate 15 feet fall hazard.  b) Worksite; 420 Maple Avenue, Swissvale, PA - Employees were removing, repairing and replacing layers of shingles on the multi-level 10x12 pitch roof without the use of a personal fall arrest systems. Employees were exposed to an approximate 40-feet fall hazards.   c) Worksite; 420 Maple Avenue, Swissvale, PA - Employees were standing and tossing materials of shingles from the top flat (no slope) main roof on the multi-level 10x12 and 12x12 pitch roof without the use of a personal fall arrest systems. Employees were exposed to an approximate 50-feet fall hazards.   The employee failed to develop and implement a satisfactory fall protection plan to ensure employees wore all parts of a full body Personal Fall Arrest System to include an anchorage, full body harness, and a connector such as a lanyard or lifeline, linking the harness to the anchorage.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  Worksite; 420 Maple Avenue, Swissvale, PA -  The portable ladder used by workers to access the third story of the roof on the residence was not extended at least 36 inches from the landing.   Employees were exposed to an approximate 30-45 feet fall hazard.  b) Worksite; 420 Maple Avenue, Swissvale, PA - The portable ladders used by workers to access the side of the house second story roof was not extended at least 36 inches from the landing.  Employees were exposed to an approximate 15-feet fall hazards.

Employees were exposed to an approximate 15-20 feet fall hazard.
