29 CFR  1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:   a. On or about May 15, 2024, at the above addressed jobsite, employees performing residential roofing operations were exposed to electrical shock while working in close proximity to 120/260V service lines.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   a. On or about May 15, 2024, at the above addressed jobsite, employees performing residential roofing operations, twelve (12) feet three (3) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about May 15th, 2024, at the above addressed jobsite, employees performing residential roofing operations on a garage , eight (8) feet nine (9) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Construction G Corp.  was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1708320, citation number 1, item number 2 and was affirmed as a final order on January 10, 2024, with respect to a workplace located at 10469 Dearlove Rd, Glenview, IL 60025.

29 CFR 1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about May 15, 2024, the employer failed to certify that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards between 12 feet, 3 inches high while performing roofing activities on a residential structure without properly using fall protection.
