29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):   (a) 75 South Park Ave, Littlestown, PA; Stiffener machine - On or about May 17, 2024, the employer had not established or identified a specific lockout tagout process for employees to safely lockout pneumatic power source(s) and also the electrical power source(s) to the Stiffener machine. The employee(s) were exposed to crushing and caught in hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) 75 South Park Ave, Littlestown, PA; Stiffener machine - On or about May 17, 2024, the guard installed at the press for the Stiffener machine did not completely enclose the press area from the operator position. Employees were exposed to crushing hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.303(b)(7)(i):Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:  (a) 75 South Park Ave, Littlestown, PA; wood panel wall production area - On or about May 17, 2024, the 240VAC electrical breaker panel for the maintenance storage area had two openings which were missing a breaker switch or blank and exposed employees to the electrical wiring inside the panel. Employees were exposed to a shock hazard.  No abatement certification or documentation required.
